What to expect?

During your initial visit you are required to fill out new patient intake forms. Please arrive 10 minutes 

prior to your appointment so you have sufficient time to complete the paperwork. This also allows the 

practitioner to see you for your fully allotted time. If you have an insurance plan please provide your 

insurance card to the receptionist so they can set you up for direct billing. After the paperwork is 

complete, the practitioner will conduct a patient history and a thorough physical assessment. Once a 

diagnosis is established the practitioner will explain your treatment plan and may render treatment with 

your consent. Expect to spend around 45 minutes in the clinic during your first visit.
